Skin of Penis getting stuck, Foreskin, prepuce, Treatment of Paraphimosis. About circumcision.

WHAT IS PARAPHIMOSIS
Paraphimosis is a condition where the foreskin when pulled behind stays back like a constricting band and even on pushing towards the tip of the penis it does not go to cover the glans penis.

In emergency Surgeon may have to operate and perform circumcision.

However, in my practice almost all the people have been able to push the skin over the glans.
This is a how to do it procedure whereby you do not need to go to a doctor

HOW TO BRING THE FORESKIN BACK IN PLACE YOURSELF WITHOUT GOING TO DOCTOR

The three simple proven steps are:
1
Get Lignox 2% from chemists shop.

2
Take cold water bath or clean genitals with cold water and soap.
Reason:
Cold water helps penis to further shrink in size. Soap cleans any infection causing microbes.

3
Apply lignox 2% on affected part.
After 5 to 10 minutes penis skin will be painless.
Now manually bring foreskin back in position.

WHAT IF THIS DOES NOT HELP
Go to qualified General Surgeon or Urosurgeon. He will do circumcision.

IMPORTANT
Read about paraphimosis and circumcision and see illustrated pictures at http://doctortells.blogspot.com

When the family strongly desires a child we recommend BBT.

BBT (Basal Body Temperature)

A more precise (though tedious) method of determining ovulation is charting your Basal Body Temperature (BBT).

When the family strongly desires a child after years of marriage, they take efforts to find the BEST time for conception. BBT is done at home and fairly reliable. Good alternative to detection of maturation done by Gynecologist or Fertility specialist using Ultrasonography.

If you want to avoid pregnancy or if you’re trying to get pregnant, knowing the time you are likely to ovulate can help you time intercourse accordingly. Many couples have had success using ovulation predictor kits, which can detect ovulation roughly 36 hours beforehand.

Rise in body temperature is conducive to implantation of zygote. It can take 24 to 48 hours for progesterone levels to build up enough to raise your body temperature. However, since the egg or ovum is only around for 24 hours, at that point it’s too late for fertilization. Charting each day’s basal body temperature will help pinpoint your likely ovulatory date and determine future patterns for predicting ovulation.

Then you can plan to have intercourse 2 or 3 days preceding ovulation. That way, the sperm can already lie in the fallopian tubes, in readiness for when an egg is released, increasing your chances of conception immensely.

HOW TO MEASURE (BBT) BASAL BODY TEMPERATURE

A special highly sensitive thermometer, the BBT thermometer, is used to measure Basal Body Temperature. Such a thermometer is graduated in one-tenth of a degree increments, and is generally inexpensive ($6-$12). Your Basal Body Temperature can be taken either orally or vaginally, with orally being the more accurate method. Whichever you choose, try to keep it consistent throughout the cycle. Also try to place the BBT thermometer the same location in your mouth or the same depth vaginally every day.

Your BBT temperature refers to the temperature of your body at rest – your temperature when you first wake up in the morning. It is recommended that you take your BBT first thing upon awakening, after at least three to four hours of sleep, before doing anything else (or your temperature will fluctuate), as well as avoid eating or drinking.

Variation in Temperature

Your temperature can rise up to one-tenth of a degree for every 30 minutes taken later than normal and fall one-tenth of a degree for every 30 minutes taken earlier than normal. If your temperature wasn’t taken at the normal time, adjust it using this rule.

Your temperature can also be affected if you have a fever,

if you didn’t get a good night’s sleep,

if you had an alcoholic drink the night before,

if you experienced a jet lag

and by changes in sleeping habits.

Ovulation Calendar – This free ovulation calendar can help you determine your most fertile days.

Rhythm method/Calendar Method for Birth Control
Ovulation Calendar helps you predict your most fertile time of the month (ovulation) so that you can achieve pregnancy. Enter your menstrual cycle information (the start date of your last menstrual period, your menstrual period length and your luteal phase length). Then simply click on “View Calendar” to view your personal ovulation calendar.

What is ovulation?
Ovulation is the release of mature ovum, which is ready for fertilization.
When spermatozoa swim through the uterus and enter in the Fallopian Tube one spermatozoon (sperm cell) succeeds in mating with the female egg cell, the ovum.

Why should I know about ovulation?
Those who want to avoid pregnancy, that is want to know when it is safe to do sex can benefit using calendar method, also called as rhythm method.

Who cannot use the Calendar Method?
Those girls, women who have irregular menstrual cycles can not rely on Calendar Method.

Who can use Calendar Method for choosing safe days for sex?
Calendar method is applicable for those women who have REGULAR menstrual cycles. If you have regular 26 days, 30 days or 28 days cycle the calendar method is useful.

It is important to know when ovum is available for fertilization. In entire one menstrual cycle only one ovum is liberated.
If your menses are regular than in 12 months only 12 ova will be liberated. (One Ovum per month.) This means if you are having sexual intercourse for 365 days you only have 12 chances of getting pregnant.
One female egg cell (Ovum) is liberated from the ovary 14 days prior to date of expected period.

The days following release of ovum is called the luteal phase.

(menstrual cycle: 28 days, luteal phase: 14 days).

It is recommended that you record your menstrual periods properly it will help you to more accurately predict your likely ovulatory date.

The ovulation calendar at this site http://www.ovulation-calendar.net/
gives option of gender preference, meaning whether you wish to have baby boy or girl.

Boy or Girl: Popularity beats science.
The author does not believe this to be scientifically validated. There is large following of girls and boys who believe in forecast of future based on zodiac signs despite knowing that such predictions of future are neither scientific nor accurate. Similarly, I shall not oppose the luxury of entertaining oneself with the notion of prediction of boy or girl baby.

REGARDING PREGNANCY (Name undisclosed)

” . . .
i had sex with my boyfrnd on the 23rd of this month
we did not use any precautions

i hve my cycle after around 28 days

. . . “

REPLY
The following arithmetic is to explain that your sexual intercourse (April 23, 2009) was in UNSAFE PERIOD, that is, FERTILE PERIOD.
Your periods had started on 14 th April, 2009.

Explaining the

CALENDAR METHOD

of Birth Control
http://mydoctortells.com/calendar-method-formula-picture/
Birth/Pregnancy Control by Calendar Method gives the SAFE PERIOD.
This is how your UNSAFE PERIOD is calculated by the Calendar Method.

First day of bleeding is counted as Day 1 in all the arithmetic below. Thus April 14, becomes your Day 1.
Presuming that for the last one year you have had regular Menstrual Cycles of 28 days then
your fertile period or period unsafe for pregnancy is

Calendar Method Formula

SAFE PERIOD is when pregnancy is unlikely and
UNSAFE(FERTILE PERIOD) is when pregnancy is most likely.
Dr. Koparday’s SAFE PERIOD Formula
s-19 and l-10
S:shortest cycle eg. 21 days
L: longest cycle eg. 29 days
in last one year’s menstrual cycles

S-19
L-10

where
S = shortest cycle _ in the past one year (eg. it could be 27 days.)
L = longest cycle _ in the past one year (eg. it could be 29 days.)

28 – 19 = 9
28 – 10 =18

See the box —->
In the present example I have taken shortest and longest cycle as 28 days because I do not know the variation in your Menstrual Cycle.

You asked:
Please make it simple.
Reply: The Thumb Rule
Presumption: MC is Regular 28 days or more days.
—–EASY TO REMEMBER —–
First 7 days (from the day of beginning of menses) and
Last 7 days (before next period)
is SAFE PERIOD

Taking the first day you bleed as Day 1, days 9 to 18 including Day 9 and Day 18 are unsafe days. In your case April 22 to May 1, 2009, presuming 1st day of your Menses was on April 14, 2009 (Day 1).

Meaning safe days are from Day 1 to Day 7 and Day 19 to Day 28

Note:
Safe days, include the days when you bleed (have periods).

You asked:
Im worried. wht if im pregnant? doctor please….
Reply:
What you should be doing is the Urine Pregnancy Test Click Here

Only one egg cell (ovum) is liberated 14 days before the expected period in any Menstrual Cycle.

The Calendar Method of Birth Control takes into account the normal variation in Menstrual Cycle, the life of sperm in vivo, the life of ovum in vivo.

Thus all possible days when pregnancy can happen are taken as UNSAFE or FERTILE Days (In your case Day 9 to Day 18) in a woman who has regular menses.

What is Regular Cycle?
Regular menses accepts few days variation (+ /- 2 days). The the woman can predict her next menstrual cycle date if she has regular cycle.

What is Irregular Cycle?
Irregular menses means some times Menstrual Cycle is after 2 months, sometimes after 20 days. Usually the woman herself cannot predict when she is likely to get periods.

Is this Calendar Method foolproof?
No. Calendar Method works well, but it is not assured to be 100% definite method of birth control as per scientific studies.

What is the safest method?
The method that is nearly 100 % definite for Birth Control is Oral Contraceptive Pills that are taken daily.

Doctor’s Choice
Combination of Calendar Method and Use of Condom during Unsafe Periods is my choice of recommendation for newly weds who want to postpone pregnancy after wedding.
Calendar Method Formula is
S-19 = First Unsafe Day and L- 10 = Last Unsafe Day

Why not use i pill instead of OC daily pills?
Can Emergency Contraceptive Method be used whenever sexual intercourse happens as a method of pregnancy control instead of Oral Contraceptive Pills that needs to be taken daily.

The i pill is also not 100 % sure method of Birth Control and it is not 100 % safe as it contains high dose of Levonorgestrel (Progesterone) hormone.
i pill of the morning after pill is therefore not advocated as routine contraceptive method. Hence, it is called Emergency Contraceptive Pill.

Please let me know if the Calendar Method as explained above is difficult to understand?

Do you have any additional questions or issues?
