BEST TREATMENT TO PROLONG SEX

By Dr. Ashok Koparday
Accredited Sex Therapist

Sex period is prolonged by increasing duration of three aspects of sex
I) Foreplay
II) Intercourse
III) Afterplay.

I) To prolong foreplay it is vital to enrich sex by increasing T – Trust, T – Time, T – Talk and only thenT – Touch.
The vital part of sex is blossoming of sexual mood in the sex partners. Wonderful details of this are already given above.

II) Intercourse consisting of insertion is prolonged by increasing the intravaginal latency time. This is accomplished by methods that do not use medicines and by those that use medicines. The nonmedicinal methods include the use of more than one condom, training to delay the ejaculatory reflex by exercising systematically the pelvic floor muscles, by masturbating prior to sexual intimacy, by increasing the weekly frequency of sexual intercourse, by increasing sexual excitement so as to do more than one consequent sexual intercourse thus naturally prolonging the sex period in all subsequent sexual intercourse. One of the popular medicines used to prolong sex period is the use of local anesthetics in the form of sprays, creams or addition to the lubricant in the condom, which are then marketed with the tag line extra long. Clomipramine, a tricyclic antidepressant prolongs sex period. Intravaginal ejaculatory latency is increased commonly by Selective Serotonin Reuptake Inhibitors. Clinical studies have shown that Paroxetine increases sex period seven folds. Dapoxetine is FDA approved medicine for prolonging sex period. Sildenafil and PGE5 inhibitors increase sex period by enabling repeated sexual intercourse resulting. Vacuum device, Intracavernosal injection of vasoactive drugs and penile prosthesis work by similar mechanism.
III) To enhance afterplay it is important to know that after ejaculation the guy should not uncouple, but continue thumping until she achieves her orgasm. Even after ejaculation rubbing movements are done by his pelvis against her so that the female genitals receive more and unbroken stimulus enabling her to achieve multiple orgasms. After both are sexually satiated light caressing helps deepen interpersonal emotional bonding causing richer sense of fulfillment. Playfulness, teasing, joviality and shamelessness directly increase quality and the quantity.

HOW TO PROLONG SEX PERIOD

By Rudy / wikianswers

Foreplay is the key, so start early with suggestions to create anticipation , getting emotions and desires heighten ed for the day. Ifyou plan an evening of bliss , start with a promise as you leave for work, then call during the day saying what you have planned, such as a shower or bath together, music and a massage with scented oils, etc. You can even leave little love notes for him/her to find during throughout day, increasing the anticipation. Think of each others’ desires and not yours alone.

For Women:

Foreplay should be an amorous, sensual adventure, making her needs and desires a priority and yours secondary . If you learn and practice this concept,it will help you learn to be a very caring, expert lover. You must realize women have emotional natures for the most part,so one first needs to know what foreplay is and what it isn’t , and kiss kiss, rub, tickle, and jump on is not the way to ensure a woman’s needs are met. Actually, it may achieve just the opposite; the woman will often end up frustrated and unsatisfied, and sometimes even resentful, feeling she is no more than an object for your needs.

However, there are timeswhen a ” quickie” is appropriate and mutually agreed upon. For women, sex is not just physical, it also includes her emotions as well. Foreplay starts longbefore the act of intercourse, it’s the difference between just having sex or being consumed with the entire lovemaking experience.

For women, foreplay actually starts long before you even reach the bedroom. Women need to feel loved and desirable at all times, notjust when you are ‘in the mood’, so be sure to give her affection and attention even when youaren’t aroused or hoping for ‘some action’. Many women are turned on by things most men would never even think of, suchas doing the dishes together, holding hands while watching TV, bathing the kids for her while she relaxes, etc.

Studies have proven these things can be a precursor to her being”in the mood”, but never do these things expecting to be ‘rewarded’ with sex since this will make her feel resentful for having to ‘pay’ for your attention or help.

Ask and learn her likes and dislikes, be attentive, notice and remember even the smallest things that make her passionate. Learn as many of her “erogenous areas” as possible, as well as things that put a twinkle in her eye, such as sweet smelling oils and a massage, Do not do the same thing every time, learn to be inventive andcreative so you avoid becoming boring and mundane. Use these to start the foreplay process, having her realize you enjoy and want to fulfill her needs and desires.

Foreplay is all about “how and when” to push the right emotional and physical buttons (if you don’t turn the light switch on, changing the bulb won’t make the light come on). Then, once the emotional buttons are pushed, you can slowly begin the physical aspect of lovemaking. Be diverse, but know how long to spend in one area. Not enough time equals frustration, too much time and they can become sore or numb because, no matter how many times you push an elevator button it won’t come any faster.

Women have tender and sensitive areas, so nuzzleand kiss these areas gently. And, when mutually agreed upon, use your hand/fingers forstimulation, which stimulates the vaginal lubrication, continuing on to oral stimulation (if acceptable with both of you); suckling the clitorisgives great pleasure to many women.

Once orgasms are achieved, women need to know you still need and want them, so”pillow talk” with hugs, caresses and talking, brings the momentum of the passion to a tender, loving finish for both partners.

If you make proper foreplay a habit the “Lovemaking Experience” will be fun, erotic and something you both look forward to, and won’t consider it an obligation or a duty.

For Men:

First, Don’t be in a hurry , go slowly.

Men, for the most part, are logical by nature, most become aroused by sight, sounds and scent, so learn what arouses him in these areas and you’re well on your way for setting the mood. Sexy or feminine lingerie, soft music, and perfume, and even more discreet things such as preparing his favorite meal can be very effective stimulants for men.

Nakedness, teasing and kissing triggers his testosterone, increasing his sex drive. Men’s erogenous zones are generally in the groin and pelvic area, so kissing, fondling, caressing and oral activities in these areas will heighten his passion. Many men also become aroused by gently nibbling or kissing his earlobe. If you do more than one of these at a time it becomes “ultra-pleasurable”, so you needto know when to stop and what areas to concentrate on to avoid premature ejaculation. Being creative and learning how to avoid anearly ending to the experience will prove to be a mutually and satisfying experience for both partners.

DEFINITION PE

Premature ejaculation (PE) is recognized to be the most common male sexual disorder. PE provides difficulties for professionals who treat this condition because there is neither a universally accepted definition nor a medication approved by the Food and Drug Administration (FDA). Despite these shortcomings, physicians continue to diagnose their patients with PE according to major guidelines and treat them with either behavioral therapies or off-label medications. This review focuses on current and emerging treatment options and medications for PE. Advantages and limitations of each treatment option are discussed in the light of current published peer-reviewed literature.

High Excitement
–>
Early ejaculation
—>
Nervousness, shame, disappointment of male.
incomplete, sex should be for longer time says female
—>
He tends to avoid/postpone sexual intercourse in order not to face shameful situation “I can’t do it.” “I can’t satisfy her.”
—>
Long gap between love making.
Meaning, frequency of sex is less.
It was 5 times a week, now it is 1 or 2 times a month.
—->
His excitement goes on rising. He also thinks that he has to fulfill her sexual need and cannot avoid sex.
—>
High excitement
—->

Early ejaculation
—>
vicious cycle
Thus, he is more disappointed, scared.
This causes UNTOLD suffering. Either he

is further tricked by quack or

bears the sorrow.

He compensates by pampering female partner in all other areas.

He has aches and pains. Example, he may complain of backache that causes difficulty in doing sex.

This dampens his desire.

Consequently his erection is not as it used to be.

“The size of my penis is small.”

He becomes workaholic.

Case File 3

am 28 yrs old young man, my weight is 95, i am married 2yrs back, duirng first year my weight was 75, even i had a happy sex no problem, but now a days i find some problems of early ejacuation during sex, after intercorse i get ejected with in 2 min, also i am finding my penis size is also geting smaller, please help me with this problem.

PREMATURE EJACULATION

People with ‘Early Discharge’, ‘Premature Ejaculation’ or ‘Early Orgasmic Response’, if given proper treatment get satisfactory results.

Two aspects weigh on the mind of the guy:

I.

What if I am not able to satisfy her?
(Fear that she may seek extramarital relation.)

II.

How can I prolong so that ejaculation is delayed?

IMPORTANT TIPS
Of FOREMOST importance is to know what is normal time.
Do you have realistic expectation?
If his expectation is to last for 30 minutes, he will get frustrated and would be visiting ‘n’ number of doctors.

5 IMPORTANT TIPS to stop premature ejaculation

5 Important Methods

TO STOP PREMATURE EJACULATION

 1. HAVE REALISTIC EXPECTATION
2. FEMALE SATISFACTION
3. MEDICAL METHODS TO DELAY EJACULATION
4. NON MEDICAL METHODS TO STOP PREMATURE EJACULATION
5. GO TO SEX THERAPIST

[1.]HAVE REALISTIC EXPECTATION

Know what is normal time for ejaculation. Be clear how long you wish to delay ejaculation? This is important because UNREALISTIC expectations (example 30 minutes) is common as people compare with what is shown in blue films.

[2.] HOW TO SATISFY FEMALE PARTNER To satisfy your partner is an important reason why you want to stop premature ejaculation.
1. Do not hurry to insert.
2. Do foreplay. You can even bring her to climax with foreplay alone.
3. Do after play. To satisfy female partner foreplay and after play are of paramount importance.
4. Do not just sexual intercourse, but sexual outer course too.
5. Learn and use non penile methods for female satisfaction. Female can have sexual satisfaction using non penile methods, such as her or partner’s finger, and knowing where and how much stimulation to give to get maximum sexual satisfaction. This takes away major burden off the mind of the man and treatment to stop Premature Ejaculation is easier.

FEMALE SATISFACTION AND DELAYING EJACULATION ARE TWO ESSENTIAL ASPECTS OF STOPPING PREMATURE EJACULATION. THEY ARE LIKE TWO SIDES OF SAME COIN
SATISFYING FEMALE AND DELAYING EJACULATION

[3.] WHAT ARE THE MEDICAL METHODS TO DELAY EJACULATION Use SSRI medicines such as Fluoxetine(Prozac (r)), Paroxetine (Paxil (r)). Alternately one can use TCA such as Clomipramine. Get a prescription and Doctor’s advice for these medicines. Use local anesthetic cream or spray

[4.] WHAT ARE THE NON MEDICAL METHODS TO STOP PREMATURE EJACULATION

 1. Masturbate before actual sexual intercourse.
2. Use condom.
3. Do not remove penis after ejaculation. Do rubbing action so that the vulva with clitoris is stimulated.
4. Do pubococcygeous muscle exercise couple of times a day. To do this imagine you have a strong urge to defecate, but there is no toilet around. In such a situation person contracts anal sphincter. Contract and release repeatedly. Do this for about 2 minutes every day.
5. Correct relationship discord. Understanding, indifferent or empathetic partner is beneficial in treatment of Premature Ejaculation.
6. Do not feel defeated and crest fallen if you ejaculate quickly. It is not a crime. Ejaculation quickly means your sexual arousal is high. Nothing bad in having high libido.
7. Increase frequency of sexual intercourse. This helps stop premature ejaculation. On the contrary, if you do sex infrequently (‘infrequently’ is a single word, not two words- ‘in frequently’) desire mounts resulting in premature ejaculation.
8. Use different sexual position or change sexual position during intercourse. However, there is no scientific proof to substantiate this.
9. Use which ever food, time, place, technique that works for you. For you that is the method to stop Premature Ejaculation.
10. If premature ejaculation does not trouble either of you do not trouble the trouble, or the trouble will double and trouble others too.

[5.] GO TO SEX THERAPIST if Premature Ejaculation is a major problem in your life.

[1] What is the usual time in normal healthy men from insertion of penis in vagina to discharge, also called as IELT?

You may be surprised to know that the NORMAL TIME from insertion to ejaculation is
60 seconds<–to–> 180 seconds.that is
1 minute <–to–> 3 minutes.
In the same person some times it can be
<–less than 1 minute or more than 3 minutes–>

[2] What do the Sex Therapists World over say?
Most Sex Therapists shy away or avoid telling this normal time and the shroud of mystery and consequent misconception prevails. The Sex Therapists cannot specify the time in minutes or in the number of strokes because it varies. All men are not of the same height. Would your answer be right if you were to specify, “5 feet, 6 inches” that is 5 and 1/2 feet, is the normal height of healthy male adult. This is incorrect. A man of 4 feet 11 inches could be as capable and absolutely normal as any other man in all areas of human life.
You cannot say that a man of height 6 feet is abnormal. Hence, your definition becomes tricky.
Dr. Prakash Kothari roars
Dr. Prakash Kothari’s patent answer to the question is, “If you were to measure the temperature of all people sitting here, would their temperatures be exactly 37 degrees Celsius or 98.7 degrees Fahrenheit. Similarly we cannot specify a particular time as normal time. If you label it as premature ejaculation, what is mature ejaculation, is another favorite question that Prof. Prakash Kothari asks. Dr. Prakash Kothari is hell bent on changing the word premature ejaculation to Early Orgasmic Response for good reasons. Medical Science knows conditions in which ejaculation is absent but orgasm present.
Example: Retrograde ejaculation following Trans Urethral Resection (TUR) of Prostrate.

When most of the times ejaculation occurs after one or two strokes following insertion or even before insertion than the Sex Therapist considers that he has Premature Ejaculation (PE) and he qualifies for treatment.
Note
Early Orgasmic Response is a term favored over the term Premature Ejaculation by my teacher and colleague Dr. Prakash Kothari [Premature Ejaculation]. Intra Vaginal Ejaculation Latency Time IELT is the medical term useful for scientific analysis and statistical studies.

Adverse Effects of Dapoxetine
Common Side Effects
Nausea (30mg = 8.7%)(60mg = 20.1%)
Headache(30mg = 5.9%)(60mg = 6.2%) resulting
Discontinuation of
Medicine 30mg = 4%) (60mg = 10%)

Giuliano F and team published the above study in Eur Urol Supp. 6(13):780-6, July 2007

All the above mentioned Medicines (except Placebo) belong to group called SSRI, , Selective Serotonin Reuptake Inhibitors. These medicines are used in the treatment of Major Clinical Depression, termed as Phoenix Disease by Dr. Ashok Koparday due to dramatic recovery of a person who is handicapped by Endogenous Depression when SSRI are given in adequate dose. SSRI correct the Serotonergic 5-hydroxytrymptamine (5-HT) neurotransmission in the brain.
A side effect of SSRI is to delay ejaculation.
This side effect is used as Therapy in people having Premature Ejaculation.

The benefit of the newer medicine Dapoxetine is that it has quick action unlike all other SSRI . Maximum Plasma Concentration is achieved in 1 hour. The Plasma Concentration of Dapoxetine 24 hours after taking the swallowing the medicine is less than 5% of the Maximum Plasma Concentration.
